Then scan in the FRONT and BACK of your credit card with signature and a picture ID such as a passport or drivers license into your computer. Please conceal the third series of numbers on your card when scanning for security purposes. Copy and paste this application and attach your scan to a e-mail. You may fax (1+256+262+0555) ONLY the application to us. But your credentials MUST BE SCANNED and sent via e-mail due to the shading of faxes being to dark and unreadable. If you do not have access to a scanner, please visit your local office supply or print shop. They should be able to scan and save the information to a portable disk for you making it very easy to then send.
